Ambry Genetics' tests are ordered by your medical care provider and results are reported to him or her. We are happy to work with you and your physician to make the testing process as easy as possible. If your physician is ready to order testing, this is the process:

Sample Collection

Usually a blood sample is collected at the physician’s office or a nearby lab and the sample is sent to us without any extra involvement on your part. For difficult-to-draw patients, certain alternative specimen types can be submitted depending on the test. Specimen Submission Kits are provided on request, or the sender can use their own materials approved for shipping medical specimens. We do require that a completed copy of the Test Requisition Form accompany the sample. Also we can work with your medical care providers and you to arrange sample collection and shipping if you are far from their facility and have no upcoming appointments.

Billing Information

There are three ways your testing fees may be paid.

• Institutional Billing: In this case, the clinic that sends us the sample requests we bill them, and they in turn bill you or your insurance.
• Insurance Billing: If we are to bill your insurance, you or your medical office can submit an Insurance Preverification Request in advance so we can determine the expected coverage of your test. ** Our Hardship Payment Plan applies **
• Medicare/Medicaid: If we are to bill Medicare/Medicaid, you or your medical office can submit an Insurance Preverification Request in advance so we can determine the expected coverage of your test.
• Self-pay: We offer a discount to patients who pay at the time of service. Also we can arrange payment plans.

Results

Reports are faxed and mailed to the clinic and/or physician who ordered the test. If you need a copy of your results after your physician has discussed them with you, please call us for the appropriate release form. To protect your confidentiality we will not release results to you without a signed release & a copy of a photo ID.
