Ambry Genetics' tests are ordered by your medical care provider and results are reported to him or her. We are happy to work with you and your physician to make the testing process as easy as possible. Usually a blood sample is collected at the physician’s office or a nearby lab and the sample is sent to us without any extra involvement on your part. For difficult-to-draw patients, certain alternative specimen types can be submitted depending on the test. Specimen Submission Kits are provided on request, or the sender can use their own materials approved for shipping medical specimens. We do require that a completed copy of the Test Requisition Form accompany the sample. Also we can work with your medical care providers and you to arrange sample collection and shipping if you are far from their facility and have no upcoming appointments.
There are three ways your testing fees may be paid.
Reports are faxed and mailed to the clinic and/or physician who ordered the test. If you need a copy of your results after your physician has discussed them with you, please call us for the appropriate release form. To protect your confidentiality we will not release results to you without a signed release & a copy of a photo ID.
